Piercing Requirements for Minors (Under 18)

Please Contact the Shop to find out Minimum Age for the Piercing you would like to get.

Minors must be accompanied by Parent or Guardian and both of you must bring ID.

Please read this carefully as these documents are important and required.

We require the following documents to perform any piercings on a person under the age of 18.

Two forms of ID for the Minor.

1.  Photo ID

Government Issue ID, Drivers License, Indiana State ID, Passport or Permit.

This ID MUST have a Photo 

2. Original Birth Certificate

This is a requirement there are NO exceptions.

The Birth Certificate Must be Original.

No photos on your phone, photocopies or Hospital copies.

A Birth Certificate must have the registered seal.

Identification for the Parent/Gaurdian 

Bring your current Drivers License, Government Issued ID or Passport.

Note to Gaurdians

If you have been assigned legal custody of the minor, you must also provide the original court documents assigning custody.

We cannot perform any piercings on a minor without all the above Documents.
